Frequently requested forms

Attendance and leave

  • Unpaid leave of absence (long) — More than one month
  • An employee may apply for general leave by filing a written application to the President (please refer to Article 25.5 in the collective agreement). For more information please contact Human Resources Operations Manager.

Compensation and benefits

  • - Submit claims to Manulife Group benefits for reimbursement. 
    • in place of HSA form

Health and well-being

  • Extended health claim form — Submit claims to Manulife Group benefits insurance for reimbursement.
  • - Submit claims to Manulife Group benefits insurance for reimbursement
    • In place of HSA form - no longer in use.
  • Doctor鈥檚 certificate form — The only format of medical note accepted.
  • To be completed by a physician for all sick leave absences greater than four consecutive workdays. For all absences greater than five working days, please contact the Health and Disability coordinator.

Hybrid Workers

Life events

Personal Information Change- To change your name (legal, preferred, revert back to maiden name) or update your gender designation. Email this completed for along with a copy of the reason for change (marriage certificate, legal name change document, birth certificate (where required)) and a copy of a piece of ID with your picture to hrinfo@ufv.ca

Change of Address (new online process)

To change your address with 糖心原创:

  • Log onto
    • Select Personal Information
    • Select Update Your Address
    • Follow the instructions provided
  • To change your payroll address, email hrinfo@ufv.ca to request the form

Note: You will also need to change your address with the benefit providers.

    • Select Group Plans
    • Select Group Benefits
    • Sign in
    • Select My Profile
    • Select edit contact information

College Pension Plan:

  • Log onto
    • Select My Account
    • Click the arrow next to name
    • Click on My Profile and "change" (next to address)

Municipal Pension Plan:

  • Log onto
    • Select My Account
    • Click the arrow next to name
    • Click on My Profile and "change" (next to address)

  • Log onto
    • Select Plan Members (under access my account)
    • Select Profile
